Postoperative rehabilitation of patellar fracture

After patellar fracture surgery, we often see many people with patellar fracture in the hospital, and patellar fracture brings great trouble to patients.

At this time, the "post rehabilitation" training of patella fracture is particularly important. So how to carry out rehabilitation training after patella fracture to achieve the best effect?


Let's have a look.


1. Early rehabilitation (1-2 weeks):


(1) Raise the affected limb: wrap the affected limb with elastic bandage 48 hours after operation, place it on the lower limb pad, raise the affected limb 20-30 °, promote venous return, and eliminate swelling.


(2) Quadriceps relaxation and contraction training: the affected limb should perform a thigh "tightening" action to make the quadriceps contraction and relaxation. Each group should be 40-50 times, 2-3 groups per day.


(3) Ankle joint movement: active flexion and extension of toes and ankle joints.


Ankle joint movement needs to complete flexion and extension and full range movement, 40-50 times per time, 2-3 groups per day.


(4) CPM training: During hospitalization, the CPM machine can perform continuous passive activities on the second day after surgery, starting from 45 °, once every 30 minutes, once a day.


If there is no pain, the discomfort can be increased by 5 ° every day.


(5) Patella activity: If there is no contraindication, push and pull the patella up and down, especially from the patella direction, to prevent adhesion between the patella and the articular surface, 15-20 minutes each time, 2-3 times a day.


(6) Knee flexion and extension: knee flexion and extension can be performed under the guidance of the doctor the next day after operation. The doctor holds the back of the affected knee with his hand, instructs the patient to relax, the calf gravity makes the knee joint bend, and then instructs the patient to straighten it slowly, 3-5 times, with the range of motion of the joint>50 degrees.


Increase knee flexion gradually after 3-4 days.


2. Mid term rehabilitation (3-6 weeks):


(1) Knee joint activity training: continue to maintain the previous joint activity training, and resume the normal flexion and extension of the knee joint.


(2) Strength training of quadriceps femoris: the legs are slightly apart, sit on a stool with the same width as the shoulder, add a sandbag of appropriate weight near the ankle of the affected limb, and keep it for 6 seconds after straightening the leg.


Then control the leg to descend slowly, 15 times per group, 3 times per day.


(3) Strength training: take a prone position on the bed, add an appropriate amount of sandbags to the affected ankle, bend the affected leg upward until the end of the knee movement, bend forward until the end, and then slowly extend to the distance a few seconds after the end of the strength.


15 times a day, 3 times a day.


(4) Control system training of knee joint:


① Take a prone position on the bed, bend your knees to 90 ° and keep it. You can clip a bag of paper towels or a deflated basketball with large volume changes between your legs, and keep it for 2 minutes, 5 times a day.


② Choose one of your own fixtures with appropriate height, such as railings. Open your legs the same width as your shoulders. Hold the railings with your hands. Keep your knees bent and squatted for a few seconds, then straighten your knees and stand up. Be careful not to use your knees more than your toes. Each group has 10 times, three groups a day.


The angle analysis of knee joint bending ability can be mastered according to the actual needs of our health. After some improvement, we can gradually increase the theoretical angle research of bending and the time of each static squat of the enterprise.


(5) Walking training: walk according to the correct gait.


The walking steps include: in the standing position, the center of gravity reaches the side leg, the affected side leg, and the center of gravity reaches the affected side leg. Make sure that the body stands firmly, and then take the side leg.


Note the placement of the left and right feet (stride and step frequency).


(6) Up and down stairs training: select the stairs with handrails.


When going upstairs, first step on the healthy side leg on the step, the affected leg always kneels, and then step on the affected leg with the healthy side leg straight.


When going down the stairs, first step out of the affected leg, the healthy leg is behind the knee, then the healthy leg bears weight, straightens the affected leg, and goes down the stairs.


Look forward and keep your waist straight.


3. Late rehabilitation


(1) Jogging training: After the recovery of knee joint activities, jogging training can be carried out to improve the endurance of lower limbs, and jogging for 20-30 minutes every day.


(3) Muscle strength training: The training teaching method is as above, and its resistance should reach our normal standard to strengthen the muscle strength of lower limbs.
